Spondylolisthesis is a condition characterised by instability in the spine, where one vertebra slips out of its normal position, potentially exerting pressure on nearby nerves and causing lower back pain or leg pain.
Distinguishing Spondylolisthesis from Spondylosis
Both spondylolisthesis and spondylosis can lead to lower back pain, but they differ in their underlying causes and characteristics. Spondylosis typically involves stress fractures or cracks in the spinal bones and is commonly seen in young athletes.
Spondylolisthesis, on the other hand, occurs when one vertebra slips out of place relative to the bone below it. This displacement can result from degenerative conditions, such as the deterioration of the disc between vertebrae and the wear and tear of facet joints. Regardless of the cause, when a vertebra slips out of its proper position, it may place pressure on the bone below it.
While some cases of spondylolisthesis may be asymptomatic, it can cause leg pain when it leads to compression or pinching of nerve roots within the spinal canal. The compression or pinching occurs when the vertebrae shift out of alignment or reduce the necessary space for the nerves.
Prevalence of Spondylolisthesis
Spondylolisthesis and spondylolysis affect approximately 4% to 6% of the adult population. Many individuals with spondylolisthesis remain asymptomatic and may live with the condition for years. Degenerative spondylosis is often linked to aging and is more common in individuals over 50, with a higher prevalence in women than men.
Causes of Spondylolisthesis
Overextension of the spine, particularly in young athletes, is a common cause of spondylolisthesis. Genetics also play a significant role, as some people are born with thinner vertebral bones. In older adults, the wear and tear on the spine can lead to this condition.
Symptoms of Spondylolisthesis
Not all individuals with spondylolisthesis experience symptoms, and many may not even be aware they have the condition. When symptoms do occur, lower back pain is one of the most common. This pain can radiate into the thighs and may be accompanied by stiffness in the back. Individuals with spondylolisthesis may find it challenging to walk or stand for extended periods and may experience pain when bending over.
Diagnosis of Spondylolisthesis
To diagnose spondylolisthesis, a spinal X-ray is typically performed to assess whether the vertebrae are misaligned. Treatment recommendations are based on the patient’s age, symptoms, and the severity of the condition. In mild cases, bed rest and physical therapy may be recommended. Surgery or injections are reserved for more severe and symptomatic cases.
