Discectomy Surgery in Max Hospital Delhi
Discectomy is a minimally invasive surgical procedure performed to remove the damaged portion of a herniated disk in the spine. This surgery aims to alleviate the pressure on spinal nerves caused by the herniation, relieving associated pain and discomfort that can radiate to the arms and legs.
When is Surgery Needed?
Discectomy becomes necessary when conservative treatment options, such as physical therapy or steroid injections, fail to relieve symptoms after six to twelve weeks. Surgery is recommended if there is difficulty standing or walking due to nerve weakness, or if pain radiates to the buttocks, legs, arms, or chest.

Diagnosis:
Before the surgery, a thorough medical history is taken, and specific tests are conducted, including routine blood tests, liver and kidney function tests, urine tests, X-rays, CT scans, and MRI. Elderly patients and those with comorbidities may undergo a detailed assessment of heart function.
Treatment:
Discectomy involves five main steps and typically lasts 1 to 2 hours. The steps include preparing the patient, making an incision, performing laminotomy (opening of the lamina), removing fragments of the herniated disc, and closing the incisions. The surgery can be performed using a fluoroscope for guidance, and it may involve an open discectomy where an incision is made in the middle of the back.
Who Are the Best Candidates for Surgery?
Candidates for discectomy include those with neurological deficits causing muscle weakness, persistent pain refractory to conservative care, failed conservative therapies, predominance of radicular pain, disc herniations resulting in cauda equina syndrome, and progressive or new motor deficits.
Who Should Not Consider Surgery?
Contraindications for discectomy include concomitant pathologies such as infection or tumor, segmental instability or vertebral fractures, spondylolisthesis, or disc fragments compressing the cord medially.
How to Prepare for Surgery:
Preparation involves discussing and signing consent forms, providing medical history information, and stopping blood thinners at least seven days before surgery. Nicotine and alcohol use should be stopped one week before and two weeks after surgery to avoid bleeding and healing problems.
Possible Complications:
Complications associated with discectomy include bleeding, infection, leaking spinal fluid, blood clots, allergic reactions to anesthesia, and the possibility of short-term relief requiring additional surgery.
Care After Surgery:
Recovery after microdiscectomy is generally quick, but precautions are necessary. Patients may face difficulty moving initially and might need assistance with walking and urination. Pain management, avoiding specific movements, regular movement to prevent stiffness, and following postoperative instructions are essential for recovery.
First 24 Hours:
- Patients may experience difficulty moving and might require assistance.
- Hospitalization may be required for some patients, while others may be sent home the same day.
Two to 14 Days:
Patients may experience sleepiness, fatigue, and pain.
- Pain medication should be taken as prescribed.
- Avoid certain movements, such as bending at the waist, lifting heavy weights, and twisting the back.
- Regular, slow movement is encouraged to prevent stiffness.
- Showering is usually allowed within 72 hours post-procedure.
- Contact the doctor immediately in case of specific symptoms or complications.
Contact Doctor Immediately If:
- Loss of consciousness
- Pain that doesn’t diminish with medication
- Calf pain or swelling in the legs
- Bleeding through the bandage
- Fever above 101°F
- Signs of infection
- Weakness or tingling
